TOURIST:
  1. Valid Passport at least six months from date of application along with previous passport if any.
  2. Two application form duly filled and signed by the applicant.
  3. Two recent passport size color photographs.
  4. Hotel confirmation.
  5. Covering letter from applicant stating purpose of visit.
  6. Covering letter from applicants company/organization stating detail of the applicant and confirming the employment status of applicant along with company profile and proof of registration.
  7. Proof of sufficient fund i.e. bank statement for last six months property paper.
  8. Income- tax return for last 3 years.
  9. Confirmed return/onward air ticket in original.
  10. Foreign exchange endorsement on passport.
  11. Sponsor either in Peru or India has to be produced the financial Status, employment proof and source of income or sponsor should give an affidavit duly notarized from Notary Public that takes all responsibility including lodging boarding of the applicant.
ADDITIONAL REQUIREMENTS FOR BUSINESS VISITORS:
  1. An invitation letter from business associates in Peru (letter of Guarantee should be in Spanish language dully attested by Notary Public in Peru.
  2. Company profile and Registration Certificate of Company .
  3. Covering letter from company stating purpose of visit
  4. Recommendation letter from chamber of commerce.
